Perhaps best 100 mg HPMC 4000 in a small tarred (with pestle and possibly spatula) mortar and mix the remaining powders. Then you can add 1.0 g propylene glycol and try to mix the whole. Then one adds very gradually under well mixing physiologically serum, which one takes from an ampoule, up to 5.0 G. Given the small volume delivered, the preparation will be consumed in a short time. I would therefore not add a preservative. Propylene glycol fulfills this function to a certain extent.
