In Our hospital we were asked to make a enema with metronidazole and lidocaine HCl

In patients with a seam detachment after transanal resection of rectal tumors, doctors would like to prescribe a enema with metronidazole (500MG/100ml) enl lidocaine HCL (200mg/ml). From the formula of the Budesonideclysma (here on the website), I have already compiled a formula: -Metronidazole 500mg -Lidocaine HCL 200mg -Carbomer Gel TMF 16g -Aqua Conservans AD 100ML The metronidazole is difficult to dissolve in water-but the powder dissolves almost completely-can I add anything to improve it? The lidocaine HCl was perfectly solved. The pH is about 6. So here I don't think you have to change anything. Are there any other tools that should/can be added? As a shelf life I would like to take 6 months. But if I consult the ' Pharmaceutical magazine for Belgium, nr ° 1, 2010 ' then a non-validated formula, aqueous solution with preservative only 1 month would be sustainable! Can you advise me here?

Answer

I know that lidocaine. HCl an exception makes at some reactions Kationogen-anionic. Whether this is also here so can only show the time. Personally, I would prefer the use of lidocaine base. If metronidazole dissolves slowly then this is due to the lack of attention to the particle size. After all, 500 mg of metronidazole dissolves in 50 ml of water.

When strict hygienic rules are applied in the preparation, the preparation can be granted a shelf life of 6 months. It is self-evident that it is assumed that the enema is delivered in unidosis packaging.
