We got following prescription for ear drops: Clioquinol 100mg Triamcinolone acetonide 10mg polyethylene glycol 400 AD 10ml <P> prepared a sterile solution

I find nothing back on the solubilities of these two and if they do not dissolve, is it then necessary to sterilize because in that case the solution can only be used for the outer ear (or is that simply suggested)?

Answer

I suspect that these ear drops are intended for the outer ear because of the presence of PEG 400. The solubility of the prescribed medicinal products in PEG 400 will not be found anywhere? So easy to try away. I would gamble that they solve.

for the preparation of sterile ear drops I would have 15 ml PEG 400 (covered with aluminum foil in a beaker, remember the practitic!) sterilize at 150 ° C in a hot air oven, for 90 minutes together with the weighed amount of dilution in Lactose of Triamcinolone acetonide, packaged in a powder folding paper from aluminum foil. (A little imagination!) At also a mortar and pestle, wrapped in aluminum foil. For the packaging I would buy a pre sterilized zentropvial.
I have no certainty about sterilization by heat of clioquinol. I should try it to see if that doesn't discolour??? But given it is an antiseptic, there may be less absolute need for sterilization.

after sterilization, allow all material to cool, then weigh the clioquinol in the sterilized mortar and then mix with the sterilized triamcinolone acetonide. Then rub with +/-5 ml of sterilized PEG 400 and transfer it into the zentropvial. The mortar rinse with sterilized PEG 400 and put on weight with PEG 400. The mass of 10 ml = 11.3 g.

by the fact that we have to use a dilution the preparation becomes so who such a suspension. If you have a very sensitive balance, the possibility is to weigh 10mg triamcinolone acetonide.
