What is the formula of ' PHOSPHATE SYRUP ' and how do you make it?

This phosphate syrup is intended for a person who has just got a new kidney.

The phosphate syrup, which I know as a phosphate source, originates from the Department of Orthopedics of Pellenberg and has the following composition:
R/Na2HPO4 76g
KH2PO4 23 g
Sorbitol 188 g
after saccharin 160 mg
Water purified to 400 ml
I fear that there may be a problem in searching for KH2PO4, which is used to not have everything in it. Perhaps to submit this formula to the doctor for a moment, given the special application.
