A physiotherapist asks if we can prepare diclofenac 8% in gel for him.

It would be used for phonoforesis and iontoforesis. The article where he draws his inspiration, recommends this formula:
R/sodium diclofenac 8g
propylene glycol q.s.
VanPen base AD 100g
Do you have a suggestion by what we can replace the VanPen base?

Answer

The composition of the Vanpen base is as follows:
cetostearyl alcohol 1.5 g
stearic acid 5g
glycerol monostearate 5g
Isopropyl 12.5 g
Lecithin: isopropyl myristaat 1:1 Solution 7.0 ml
Butyl Hydroxytolua 10% in Ethanol 94 ° 1ml
simethicone 2g
urea 2g
potassium sorbate 200mg
NaOH 30% solution 0.25 ml
PEG-40-stearate 5g
NaEDTA 5% solution 2 ml
purified water up to 100g
the lecithin solution can be prepared by mixing: 0.02 g Sorbinezuuur, 5g soya lecithin and 5g Isopropylmyristaat

This is a fairly complex compound. The most typical is the presence of the lecithin isopropyl myristate to increase transdermal absorption. This is echoing the PLO gel.

I would take the CETOMACROGOLCR è me TMF as a starting point to which I would make the following changes. Liquid paraffin and if possible petroleum jelly (see end result) I would substitute Isopropyl (IPM) or oleyloleate (Cetiol), which are as carriers for medicines. In addition, I would add 10 à 15% propylene glycol. The addition of the lecithin isopropyl myristate is recommendable and realizable. After all, Lecithin is available.
