Solution for local anesthetic in the nose cocaine HCl 2g adrenaline 0.1% 4dr Aqua AD 20ml

This preparation serves to anesthetic in preparation for nasal surgery. It is intended that the doctor will mix this solution with Fluimicil precisely before treatment. How do we proceed in terms of the stability of this preparation? Which preservative to add? Store outside light?

In The USP is described: cocaine HCl 4g on 100ml water base (with Tetracaine HCl 1g, adrenaline1/1000 25ml, Benzalkonium 10mg, disodium EDTA 6.4 mg, NaCl 0.9% 35ml

Answer

The adrenaline solution described in Pharm. Belg VI contains 0.1% sodium bisulphite as an anti oxidance and this concentration applies to the total volume of the solution. This means that the prescribed solution should be added 20 mg of nametabisulfite.
Cocaine HCl 2g
Adrenaline amp (sterile) 0.8 mg/ml 0.25 ml
after metabisulfite 20mg
Benzalkonium chloride 2mg
Aqua PRO INJ. AD 20ml
The adrenalin solution of the Pharm. Belg VI does not contain EDTA. If you wish you can add this ingredient. It will not be detrimental. Only the value of the pH of the solution should be located near the value 3. Adjust if necessary. A solution 1 to 100 should be prepared from benzalkonium chloride.
